Position Summary
We are seeking a Nursing Supervisor to join our office team. This position supports essential functions that follow and maintain agency standards in accordance with Agency Policies and Procedures, as well as state and federal regulations.
The Nursing Supervisor reports directly to the Primary and Senior Nursing Supervisor and supports, while indirectly reporting to, all HiHealthCare leadership.
This role supervises and conducts field staff visits in the community to ensure clinical execution of agency policies and procedures. The primary function of this position is to support the Clinical Team by providing oversight and collective feedback on clinical processes. The Nursing Supervisor serves as a liaison between office-based healthcare staff and field staff to ensure continuity of care and adherence to agency standards.
Responsibilities
- Conduct supervisory visits to assess care and services provided by Agency staff, address patient and family needs and concerns, and ensure Agency equipment is functioning properly.
- Establish and update Plans of Care in accordance with OASIS-mandated guidelines.
- Assist with assessment, planning, implementation, and evaluation of patient and family/caregiver care.
- Assist in opening new cases, conducting initial assessments and reassessments of clients/patients and caregivers, and ensuring all clinical documentation is complete.
- Ensure care delivery complies with regulatory and contractual requirements and communicate with other healthcare providers regarding patient progress.
- Provide care coordination for clients receiving nursing services, including direction to case managers and clinical teams related to patient care delivery.
- Maintain positive working relationships with:
- Patients and patient families
- Field staff
- Administrative and home office staff
- Insurance providers and case managers
- Physicians
- Community partners
- Provide collective feedback and assist in the development and implementation of clinical and administrative policies and procedures.
- Assist the Clinical Team with the development, implementation, and evaluation of new clinical programs.
- Assist in the investigation and evaluation of incident reports, employee injury reports, and client or staff complaints, reporting all required information to the Clinical Team.
- Assist with Agency quality assurance (QA) chart and file audits through regular assessments, patient visits, and chart reviews.
- Cover home care shifts as needed.
- Be available for on-call coverage after hours, weekends, and holidays as scheduled to support staff and provide patient services.
- Assist the Clinical Team with staff training, orientations, evaluations, compliance, and counseling.
- Assist with new hire interviews.
- Assist with developing, implementing, and evaluating orientation and educational programs.
- Participate in weekly team meetings and other mandatory in-services (in person or virtual).
- Perform other duties as assigned.
Requirements
- Minimum 1 year experience in Home Health Care or Acute Care
- Current State of Hawaii Registered Nurse License
- Current Basic Life Support and First Aid Certification
- Home Health Care Management experience preferred
- Case Management experience preferred
- Valid Driver’s License
- Reliable mode of transportation
- Pass drug screen and criminal background check
